Phalaenopsis tetraspis adalah salah satu spesies tumbuhan epifit dari famili Orchidaceae (suku anggrek-anggrekan). Tumbuhan ini berasal dari bioma beriklim tropis basah di wilayah Andaman dan Kepulauan Nikobar, Sumatra barat laut. Nama ilmiah Phalaenopsis tetraspis diperkenalkan oleh seorang botanis bernama Heinrich Gustav Reichenbach pada tahun 1870.
